The Division of Supportive Services supports FirstMed’s goal of developing a community-based health care system by supporting adults that have been impacted by trauma and helping them navigate behavioral health services to achieve self-sufficiency with the assistance of collaborative and integrative case management and community support. The DSS supports the NCCY by working with adult members of the family to achieve stabilization within the family unit. Case management provided is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
FirstMed also provides support to adult and child victims of a crime. If you are a primary or secondary victim of a crime, you may have to cope with challenges you never expected to face.
