- You must first make a medical appointment at one of our three clinics.
- Attend your first appointment and meet your health care professional.
- Make another appointment with us, either for the same or different reason you came the first time.
- You may also use our Community Health Wellness classes as a qualifying “visit”.
- Group therapy is also considered a qualifying “visit.”
- Psychotherapy does NOT qualify as a visit.
- Repeat these steps 5 times and on your 6th visit you’ll get a $500 visa gift card!
Remember: You will only be able to participate if you have never been a patient at FirstMed Health & Wellness. This includes all three locations in the Las Vegas area. You must keep and attend all 5 appointments UNLESS there is an emergency that requires cancellation, and that cancelled appointment MUST be rescheduled at the time you are making the cancellation.
